Work Environment

This position is primarily onsite as you will be delivering home care services directly within the residences of our patients. Flexibility is essential, as treatment schedules may vary, and weekend availability may be necessary. You will work closely with a dynamic interdisciplinary team comprised of physicians, support staff, and other healthcare professionals to ensure comprehensive and effective patient care.

Key Responsibilities

• Conduct thorough examinations and evaluations of patients to assess their functional limitations and disabilities.
• Develop and implement individualized treatment plans based on patient assessments, ongoing evaluations, and established best practices.
• Educate patients, families, and caregivers on treatment protocols and home exercise programs to foster continuity of care.
• Collaborate with physicians and healthcare providers to ensure complete and coordinated care throughout the patient's treatment journey.
• Accurately document patient treatments, ongoing assessments, and any necessary changes to treatment plans, adhering to all regulatory and professional standards.
• Participate in community outreach programs to promote physical health and therapeutic practices.
• Supervise and provide guidance to physical therapy assistants and students, sharing your expertise and improving overall team competency.
• Engage in professional development through continuing education and participation in staff meetings.

Patient Population / Client Focus

The patients you'll work with in this role include individuals across a spectrum of orthopedic conditions, recovery from surgery, trauma, chronic pain, and geriatric care. Your ability to effectively assess and address each patient's unique needs will be critical in delivering effective care and improving their functional abilities.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current active state license in Physical Therapy.
• Basic Life Support (BLS) certification.
• Effective written and verbal communication skills, with a strong focus on patient interaction.
• Proficiency in electronic documentation systems and basic computer skills.

Preferred Experience

While a minimum of two years of relevant clinical experience is required, candidates with experience in home health care, outpatient physical therapy, or specialized orthopedic services will be prioritized. Familiarity with treatment modalities such as therapeutic exercises, manual therapy, and patient education techniques will also be advantageous.

Compensation and Benefits

We are pleased to offer a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
