Work Environment\n

This position is a full-time remote role, providing flexibility to manage a schedule that best suits our clients' needs. Standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evenings and weekends as necessary. You may also be required to be on call to address urgent client matters or provide support outside of regular hours. Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, evaluating emotional, mental, and social factors to inform care plans.

• Develop personalized care plans in collaboration with clients and their families to address specific goals, preferences, and challenges.

• Provide counseling services to seniors and their loved ones, supporting them through life transitions such as health changes, loss of independence, or bereavement.

• Act as an advocate for clients, connecting them to community resources that offer essential services and support.

• Collaborate with an interdisciplinary team, including nurses, caregivers, and other healthcare professionals, to enhance the quality and continuity of care.

• Conduct regular home visits or virtual check-ins to monitor clients’ well-being, assess living conditions, and ensure safety.

• Facilitate family meetings to improve communication among family members regarding care decisions and conflict resolution.

• Educate clients and families about local support groups, available resources, and social engagement opportunities.

• Maintain thorough records including client assessments, care plans, and progress notes, utilizing electronic health record (EHR) systems for documentation.

Qualifications\n

Compensation and Benefits\n

This position offers a competitive salary range of $60,000 to $100,000 annually, commensurate with experience and qualifications.
