Key Responsibilities\n

• Maintain accurate and thorough documentation reflecting your clients' progress, including timely client notes and mileage reports, ensuring compliance with regulatory standards.

• Facilitate skill-building sessions with clients, assisting them in mastering daily living skills such as shopping, meal planning, and cooking, tailored to their individual readiness and needs.

• Monitor and report on clients' emotional states and overall wellbeing, actively implementing agency safety protocols and acting as a positive role model to encourage client progress.

• Provide necessary support in medication management, including prompting clients to take their medications, ensuring timely refills, and addressing adherence to prescribed regimens.

• Offer transportation support for clients, both in personal and agency vehicles, and provide guidance in utilizing public transportation options for attending medical appointments.
\n
• Ensure that households maintain adequate supplies and assist in obtaining and transporting these essentials as needed.
\n
• Achieve client satisfaction by delivering services that meet and exceed established care standards while actively working toward individual goals.

• Collaborate with interdisciplinary teams to foster effective communication, ensuring all team members are informed about client needs and progress.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor, you will work with a culturally diverse clientele, including individuals facing various mental health challenges and those requiring supportive counseling in a community health setting. Qualifications\n

• Possession of an active and unrestricted LCSW, LMFT, LPCC, or equivalent license to practice in California is mandatory; applicants with associates licenses cannot be considered due to billing regulations.

• Strong self-management skills, with a proven track record of working independently and without direct supervision.

• Proficiency in Microsoft Office Suite, particularly Word, for documentation and communication purposes.
