Key Responsibilities\n

• Conduct comprehensive assessments to identify clients' social, emotional, and psychological needs, utilizing standardized tools and methods to ensure thorough evaluations.

• Facilitate individual and group therapy sessions using evidence-based therapeutic approaches to guide clients through their emotional and mental health challenges.

• Develop personalized service plans in collaboration with clients, ensuring that each plan aligns with their goals and addresses specific needs related to hospice care.

• Advocate for clients’ rights to access essential resources, including financial assistance and emotional support services while addressing social justice issues pertinent to hospice care.

• Engage in community outreach, working closely with local organizations and service providers to enhance the overall well-being of clients and promote awareness of hospice resources.

• Collaborate with an interdisciplinary team, contributing to program development, evaluations, and grant reporting to strengthen service delivery and maintain organizational effectiveness.

Patient Population / Client Focus\n

The Clinical Social Worker will focus primarily on providing care and support to patients facing life-limiting illnesses and their families. Compensation and Benefits\n

The salary for this position ranges between USD 75,000 and 100,000 annually, with the potential for a sign-on bonus.
