Key Responsibilities\n

• Coordinate with Reentry Social Workers to facilitate appropriate discharge planning services tailored to individual client needs.

• Complete required statistical reports to aid in monitoring service utilization and efficacy.
\n
• Collaborate effectively with the Department of Correction to ensure timely access to necessary mental health services, secure proper housing, and ensure appropriate treatment disposition for clients.
\n
• Coordinate patient care in collaboration with other institutional programs, including leading group treatment sessions on mental health observation units.
\n
• Document all services provided meticulously in compliance with organizational standards and participate in weekly clinical supervision for professional development.
\n
• Engage actively in mental health and medical rounds as required, contributing insight to improve client outcomes.
\n
• Participate in case conferences and collaborate on treatment planning for patients receiving and being considered for mental health treatment.
\n
• Conduct mental health evaluations, crisis intervention assessments, and provide follow-up care, ensuring that clients receive continuous support.
\n
• Determine and coordinate the disposition of patients to ensure they receive the appropriate level of mental health care, tailored to their unique situations.
