Position Overview

Breakthrough Rehabilitation & Treatment Center is actively seeking a dedicated and licensed Full-Time Physical Therapist to join our team in East Brunswick, New Jersey. In this role, you will leverage your expertise to provide exceptional care to patients in our outpatient orthopedic setting, working diligently to improve their physical wellbeing and quality of life. Work Environment

The physical therapist will work onsite in our outpatient office, where you will engage with patients in a dynamic and collaborative atmosphere. Key Responsibilities

• Conduct comprehensive evaluations of patients, assessing functional limitations and disabilities to formulate and implement tailored treatment plans that align with their specific rehabilitation goals.
• Utilize evidence-based treatment protocols designed around individual patient evaluations, adjusting plans as necessary in response to patient progress and overall health status.
• Provide in-depth patient education and engage patients and their families in the rehabilitation process, ensuring they understand the treatment plan and expected outcomes.
• Collaborate with a range of healthcare professionals to enhance patient outcomes and address additional needs, formally documenting any changes or recommendations in patient care.
• Maintain accurate, timely documentation that reflects the patient's care journey, including detailed evaluation notes, progress summaries, and discharge planning.
• Ensure compliance with state and federal regulations through continual education and adherence to corporate policies related to patient care.
• Engage in facility meetings and contribute to initiatives aimed at improving accessibility and patient care services, actively participating in team discussions to eliminate barriers to access.

Patient Population / Client Focus

The primary focus will be on orthopedic patients who require physical therapy for recovery from surgery, injury, or chronic conditions. You will work with a diverse patient population, including individuals recovering from orthopedic surgeries, sports injuries, and chronic pain conditions, enabling them to regain their functional abilities and enhance their overall physical wellness.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy (PT) program.
• Current licensure as a Physical Therapist (PT) in the state of New Jersey, confirming compliance with regulatory requirements.
• A minimum of two (2) years of experience in an outpatient orthopedic setting, demonstrating a solid foundation in patient evaluations and treatment implementation.
• A valid CPR certification.

Preferred Experience

Experience working with diverse patient populations in an outpatient setting is highly desirable, along with familiarity using digital documentation systems to streamline patient records. Experience with specific treatment methodologies related to orthopedic rehabilitation is also beneficial.
