Position Overview

Breakthrough Rehabilitation & Treatment Center is actively seeking dedicated and qualified Physical Therapists to join our team at our Woodbridge, New Jersey location. This full-time position offers an annual salary ranging from USD 100,000 to 120,000, along with a comprehensive benefits package. Work Environment

This role is set in an onsite home care facility, emphasizing direct engagement with patients in a welcoming environment. Our ideal setting combines modern rehabilitation equipment with a supportive atmosphere that encourages teamwork and interaction among staff. The working hours are Monday to Friday, from 8:00 am to 5:00 pm, providing a structured schedule that allows for work-life balance while ensuring dedicated patient care.

Key Responsibilities

• Conduct thorough assessments to examine and evaluate patients, developing individualized treatment plans based on their functional limitations and goals.
• Adhere to state practice regulations, ensuring compliance with all relevant laws and maintaining professional standards in patient supervision and care.
• Utilize specific evaluation techniques, including collecting pertinent medical history and performing clinical tests, to inform treatment decisions and optimize patient outcomes.
• Implement effective treatment protocols that leverage evaluation findings and adapt based on ongoing patient assessments and evolving healthcare dynamics.
• Consult with interdisciplinary healthcare teams to modify care plans in accordance with changes in patients' conditions, ensuring timely interventions to address new challenges.
• Document patient care activities meticulously, maintaining accurate records of evaluations, treatment plans, and discharge summaries in compliance with regulatory standards.
• Manage discharge planning to facilitate a seamless transition for patients, including setting follow-up care expectations and necessary referrals.
• Participate actively in facility meetings and quality improvement initiatives, addressing barriers to patient access and supporting organizational goals for improved care delivery.

Patient Population / Client Focus

You will work primarily with a diverse patient base requiring orthopedic rehabilitation, including those recovering from surgeries, sports injuries, and chronic pain conditions. Your role will encompass evaluating each patient's unique needs, promoting functional mobility, and enabling them to achieve their personal health goals through tailored therapy interventions.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Current licensure as a Physical Therapist in the state of New Jersey, obtained through the Physical Therapy Examining Committee.
• A minimum of two (2) years of experience in an outpatient orthopedic setting.
• Current CPR certification, with up-to-date knowledge of emergency protocols and patient safety measures.

Preferred Experience

While a minimum of two years in outpatient orthopedics is required, candidates possessing advanced certifications in specific therapeutic techniques or specializations may have an additional advantage. Experience with electronic health record (EHR) systems is also preferred, as it will facilitate comprehensive documentation and improved patient management.
