Key Responsibilities

As a Physical Therapist at Confluent Rehabilitation, you will have a multifaceted role that includes:

• Directing personalized physical therapy interventions to alleviate pain, enhance mobility, and prevent disability in patients, following established care plans.
• Conducting thorough evaluations of clients' functional abilities through advanced diagnostic and prognostic assessments.
• Assuring the proper planning and execution of care by overseeing Physical Therapy Assistants in their treatment of clients.
• Providing in-home therapy services as prescribed, demonstrating expertise in therapeutic modalities such as manual exercises, traction, and therapeutic technologies.
• Developing and revising comprehensive care plans based on client assessments and performance outcomes.
• Documenting each patient's progress accurately, maintaining thorough clinical records—initial evaluations, care plans, and discharge summaries—that reflect their journey in rehabilitation.
• Engaging with interdisciplinary teams during case conferences to discuss client treatment strategies and outcomes.
• Instructing patients and their families on care protocols, therapy techniques, and equipment use to promote home exercise programs.
• Regularly reporting changes in client conditions to physicians and nursing staff and adjusting treatment plans as necessary.

Patient Population / Client Focus

Our clientele consists of individuals recovering from various injuries, surgeries, or chronic conditions, requiring targeted rehabilitation strategies. You will work with patients across different age groups, ensuring adaptive approaches are in place to meet their specific needs. Our patient populations often include older adults requiring rehabilitation post-surgery or those facing mobility challenges due to neurological conditions. Qualifications

The ideal candidate should possess:

• A Doctorate in Physical Therapy (DPT) and a valid state PT license in good standing.
• A minimum of two years of experience treating patients in either an acute care or home care setting.
• Current CPR certification and a valid driver's license with automobile insurance.
• Exceptional interpersonal communication skills, with a proven ability to engage and educate patients and their families.
