Work Environment

This position is onsite and primarily involves home care, allowing you to interact directly with patients in their living environments. Key Responsibilities

• Conduct thorough evaluations to assess patient conditions, including performing specific assessments to form a comprehensive understanding of each individual's needs.
• Develop and communicate personalized treatment plans alongside patients and their families, ensuring that their goals and desires are integral to the therapeutic process.
• Implement treatment protocols based on clinical findings, adjusting approaches as necessary to respond to patients' evolving conditions and healthcare regulations.
• Collaborate with other healthcare professionals to ensure comprehensive care, actively documenting changes in patient conditions that may require modifications to treatment plans.
• Manage all aspects of discharge planning and follow-up care, including precise and timely documentation of patient interactions, progress notes, and evaluations to enhance continuity of care.

Patient Population / Client Focus

As a Physical Therapist with Superb Life Living Physical Therapy, you will work closely with a diverse patient population, including those recovering from surgery, managing chronic pain, or requiring rehabilitative support due to various health conditions. Your role is vital in helping these individuals regain mobility, reduce pain, and improve their overall quality of life through personalized care approaches.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid Physical Therapy license to practice in the state of Florida.
• Current American Heart Association's BLS for Healthcare Providers CPR certification.
• Strong communication and interpersonal skills for interacting with patients and their families.

Preferred Experience

• A minimum of 2 years of experience in a clinical setting, preferably with a focus on home care or outpatient physical therapy.
• Experience with electronic health record (EHR) systems can be an asset.
