Work Environment\n

This position operates in a remote setting, allowing flexibility while maintaining professional standards of care. As a Remote Social Worker, you will collaborate with an interdisciplinary team, including nurses, hospice caregivers, and other healthcare professionals, to deliver tailored services to our clients. You will conduct regular assessments and meetings through secure video conferencing and other digital communication tools, ensuring that clients receive ongoing support and check-ins from the comfort of their homes.

Key Responsibilities\n

• Conduct detailed psychosocial assessments for seniors, identifying their emotional, mental, and social needs.

• Develop and implement personalized care plans in collaboration with clients and their families, ensuring individual goals and preferences are prioritized.

• Provide emotional counseling and guidance, helping seniors and their families navigate life transitions associated with aging, illness, and bereavement.

• Act as an advocate for clients’ rights, diligently sourcing community resources that enhance their access to care and service provisions.

• Coordinate with interdisciplinary teams to ensure comprehensive support across all levels of care, promoting optimal client health outcomes.

• Monitor client well-being through regular check-ins, addressing any concerns regarding their living situations or ongoing health conditions.

• Facilitate family communication, aiding in conflict resolution and ensuring family members are involved in care decisions.

• Provide clients and families with education on available community resources, social engagement opportunities, and support programs.

• Maintain robust and accurate documentation of client interactions, assessments, and care plans in compliance with ethical and organizational standards.

• Stay current on best practices, policies, and regulations in social work and hospice care to provide the highest quality support.

Patient Population / Client Focus\n

This role focuses primarily on elderly clients receiving hospice care. Our clients come from diverse backgrounds, presenting unique psychological and emotional needs stemming from chronic illness and the aging process. As a Social Worker, your expertise will assist families in understanding and addressing the multifaceted aspects of aging and end-of-life care.

Qualifications\n

• Master's degree in Social Work from an accredited institution, demonstrating advanced knowledge and skills in the field.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• At least two years of relevant experience in social work, preferably in geriatric or community-based settings.

Compensation and Benefits\n

The compensation for this role ranges from $60,000 to $100,000 annually, commensurate with education and experience.
