Key Responsibilities\n

• Conduct thorough assessments of clients to gather detailed information regarding their presenting issues, personal history, and treatment objectives.

• Deliver individual therapy sessions, as well as family and couples therapy as needed, utilizing evidence-based practices such as cognitive-behavioral therapy and mindfulness techniques.

• Create and execute personalized treatment plans tailored to the unique needs and goals of each client undergoing hospice care.

• Regularly assess client progress and adjust treatment plans as necessary to ensure optimal outcomes.

• Maintain strict confidentiality and adhere to professional ethical standards and legal regulations at all times.

• Engage in ongoing professional development to enhance clinical skills and stay abreast of current best practices in social work and hospice care.

• Build strong therapeutic relationships with clients by establishing trust and facilitating open communication, thereby enhancing the therapeutic experience.

• Provide comprehensive support and practical guidance to help clients and their families navigate the complexities of end-of-life circumstances.

Patient Population / Client Focus\n

Your primary focus will be on clients receiving hospice care, along with their families. You will work closely with individuals facing terminal diagnoses, assisting them in processing their emotions, managing grief, and enhancing their overall quality of life during a pivotal time. Your role will also include providing crucial support to family members as they confront the emotional challenges associated with loss and transition.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• An active state license as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of clinical experience, ideally within a hospice or palliative care context.

Compensation and Benefits\n
