Work Environment\n

This role is conducted in a remote setting, allowing for flexibility in your workspace while maintaining a structured work schedule from Monday to Friday, 9:00 AM to 5:00 PM. Even though the position is remote, collaboration with a team of other professionals is key to delivering holistic patient care. As part of our hospice team, you will work closely with psychologists, nurses, and other social workers to ensure the highest quality of support for our clients and their families.

Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments to understand clients' needs and strengths, followed by the creation of personalized care plans that reflect their unique circumstances.

• Utilize evidence-based techniques and therapeutic modalities to facilitate mental health improvements among clients, particularly those facing end-of-life challenges.

• Maintain meticulous clinical documentation in adherence to legal and ethical standards, ensuring that records are thorough and up-to-date.
