Key Responsibilities\n

• Establish strategic partnerships with community services to facilitate seamless aftercare arrangements for our patients and their families.

• Conduct thorough assessments of patient's psychosocial needs, utilizing effective interviewing techniques and chart reviews to inform individualized treatment plans.

• Analyze and document patient behavior and emotional conditions, ensuring accurate and up-to-date patient records in compliance with social work best practices and facility standards.

• Utilize evidence-based practices, including behavior modification techniques and supportive therapy, to intervene and guide clients through resistance and conflict, enhancing their coping strategies.

• Develop and implement comprehensive treatment plans that align with the unique values and concerns of patients and their families, revising them as necessary based on ongoing evaluations.

• Facilitate educational and therapeutic group sessions, meticulously documenting patient progress and outcomes to optimize therapeutic interventions.

• Participate actively in multidisciplinary team meetings to provide insights into patient progress and collaboratively develop strategies for ongoing care.

Patient Population / Client Focus\n

Your primary focus will be on patients enrolled in hospice care, a context that requires sensitivity to the emotional and psychological aspects of terminal illness. You will work closely with patients, their families, and a diverse healthcare team, necessitating a deep understanding of grief, loss, and the transition process associated with end-of-life situations. Your role is critical in ensuring families receive the support and resources they need to cope with these challenges effectively.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related mental health field is required.

• Currently licensed as an independent Licensed Clinical Social Worker (LCSW) in California.

• Basic Life Support (BLS) and CPR certification are mandatory.

Clarity Reached Counseling Services, LLC offers a competitive annual salary ranging from $120,000 to $130,000 for this full-time position.
