Key Responsibilities

• Conduct thorough evaluations of patients through detailed history-taking and specialized assessments, ensuring a deep understanding of their individual needs and health conditions.
• Communicate evaluation findings and treatment plans clearly with patients and their families, actively involving them in setting achievable rehabilitation goals.
• Implement tailored treatment protocols based on evaluation results, patient presentation, and evolving healthcare trends, demonstrating a commitment to evidence-based practice.
• Collaborate with multidisciplinary healthcare teams to address complex patient needs and adapt care plans as necessary based on ongoing progress and changes in health status.
• Ensure meticulous documentation of patient interactions, including initial evaluations, daily progress notes, and discharge summaries, to maintain clear communication and track patient outcomes.

Patient Population / Client Focus

Qualifications

• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.
