Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of individuals, families, and groups, utilizing well-established clinical social work and counseling methodologies to inform treatment plans.

• Engage in initial patient assessments to determine care needs and triage accordingly, ensuring timely and appropriate interventions.

• Collaborate effectively with interdisciplinary teams, providing clear documentation and participating in patient rounds to discuss treatment progress and updates.

• Employ evidence-based practices in your therapeutic approach, including but not limited to Cognitive Behavioral Therapy (CBT), to support the completion of individualized service plans for clients.

• Deliver case management services, actively consulting with both hospital and community resources to coordinate holistic care for patients.

• Commit to ethical documentation practices, ensuring timely updates that reflect patient progress according to departmental standards.

As a licensed mental health counselor at Trinity Health, you will work primarily with a diverse outpatient population that includes individuals from various backgrounds and age groups, including children, adolescents, and adults. Compensation and Benefits\n

The compensation for this role ranges from USD 80,000 to 100,000 annually, commensurate with experience and credentials.
