Key Responsibilities\n

• Conduct comprehensive assessments to evaluate and meet the varied needs of patients, ensuring clear, effective plans are developed for ongoing support.

• Facilitate open communication between clients, families, guardians, and healthcare team members regarding treatment progress, demonstrating dedication to collaborative care.

• Organize and manage a tentative schedule of appointments, home visits, and related observations to monitor client progress and adjust treatment plans as necessary.

• Coordinate home visits as required to complete in-home observations, perform chart reviews, staff interviews, and analyze data effectively.

• Ensure the effective implementation of treatment program plans, contributing to each patient's continual improvement and empowerment.

• Participate in interdisciplinary team meetings, contributing insight and updates on individual client progress while collaborating with fellow healthcare professionals.

• Create detailed monthly and quarterly progress notes to comprehensively document treatment advancements and assessments.
\n
• Organize and facilitate training for team members to ensure competence in implementing program plans and documentation standards.
\n
• Provide follow-up interviews and observations to offer continued support aligned with individual client needs.

• Maintain clear, accessible records that guarantee relevant information is readily available to all involved parties.

• Act as a liaison with the public and the mental health community to support development goals, fostering engagement with stakeholders.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is mandatory.

• Possession of an active and unrestricted Professional Counselor (LPC) license in the state of Texas.
