Key Responsibilities\n

• Conduct thorough assessments to tailor treatment plans that meet the unique needs of each patient based on their mental health status and personal goals.

• Facilitate effective communication among supervisors, direct care staff, and family members by providing updates on individual patient progress and needs.
\n
• Develop and maintain a detailed schedule of appointments, home visits, and ongoing evaluations to monitor patient advancement in compliance with treatment objectives.
\n
• Coordinate and execute in-home visits as needed, including chart reviews and staff interviews, to ensure comprehensive understanding of patient circumstances.
\n
• Analyze and document the effectiveness of program plans, ensuring they are implemented correctly and adjusted as necessary.
\n
• Actively participate in Interdisciplinary Team Meetings, collaborating with a variety of health professionals to enhance patient care strategies.
\n
• Complete comprehensive monthly and quarterly progress notes to track patient development and modify treatment plans accordingly.
\n
• Organize training sessions to ensure that team members are fully competent in implementing program plans and effectively utilizing documentation tools.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations to foster patient well-being and adherence to treatment goals.
\n
• Maintain accurate and organized patient records in compliance with healthcare regulations, ensuring that vital information is accessible to relevant stakeholders.

• Serve as a liaison between the organization and the broader mental health community, helping align development goals with community resources.
\n
• Assist in the creation and refinement of training materials aimed at enhancing program effectiveness and outcomes for individuals served.

Patient Population / Client Focus\n

In this role, you will work with a diverse client base, including both adults and youth, addressing a wide range of mental health issues such as anxiety, depression, trauma, and substance misuse. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Tennessee is mandatory.

• A minimum of two years of counseling experience in a clinical context is essential.

• A specialization in family therapy, couples therapy, trauma, or substance misuse is highly preferred.

• An in-depth understanding of legal and ethical standards in mental health counseling.

• Exceptional interpersonal and communication skills, with the ability to build rapport across diverse client populations.

• Proven ability to collaborate effectively within a healthcare team to achieve optimal patient outcomes.

Preferred Experience\n

• Familiarity with Electronic Medical Records (EMR) software and telehealth technologies is advantageous.

Compensation and Benefits\n

The salary for this position ranges from $90,000 to $100,000 annually, depending on experience and caseload availability. In addition to competitive compensation, we offer a flexible work schedule, promoting a healthy work-life balance.
