Key Responsibilities\n

• Conduct comprehensive intake assessments to gain insight into clients' goals and preferences, guiding the creation of individualized care plans.

• Collaborate closely with interdisciplinary teams, including physicians, occupational therapists, and psychologists, to develop tailored rehabilitation programs that cater to client's specific needs and challenges.

• Maintain detailed case files and documentation of client progress, treatments, and any modifications made to care plans to ensure optimal outcomes.

• Regularly monitor and track client progress and adjust care plans as necessary, employing evidence-based methodologies such as cognitive-behavioral therapy (CBT) and dialectical behavioral therapy (DBT).

• Engage with clients' family members and support networks to facilitate a holistic approach to treatment and to ensure all parties are aligned with treatment goals.

• Provide ongoing support during clients' participation in job training and occupational placement programs, addressing any challenges that arise promptly.

• Assess and document client needs and make informed recommendations for further resources or referrals as identified.

Patient Population / Client Focus\n

The Licensed Professional Counselor will work with a diverse range of clients, including those experiencing mental health disorders, mood disorders, and other related challenges that impact their daily functioning. Qualifications\n

• A Master's Degree in Counseling, Social Work, Psychology, or a related field is required.

• A current Licensed Professional Counselor (LPC) credential is mandatory.
