Key Responsibilities

• Direct and supervise physical therapy treatments aimed at preventing disability and relieving pain through a combination of therapeutic interventions and evidence-based practices.
• Assess and evaluate clients' physical conditioning using diagnostic tests to develop individualized treatment plans catered to their specific orthopedic needs.
• Oversee and collaborate with physical therapy assistants in ensuring that care plans are effectively implemented and adjusted as necessary.
• Conduct ongoing evaluations of patient progress, documenting changes and communicating updates with physicians and case managers to refine treatment strategies.
• Facilitate patient education, instructing clients and their families on exercises, therapeutic techniques, and the use of rehabilitation devices to promote independence and recovery.
• Participate actively in interdisciplinary team meetings to develop and evaluate comprehensive care plans for optimal patient outcomes.
• Maintain meticulous clinical records, including initial evaluations, progress notes, and discharge summaries, to ensure compliance with regulatory standards.
• Utilize manual therapy techniques, physical agents, and exercise modalities to enhance rehabilitation outcomes and improve functional abilities.
• Provide home evaluations and make necessary adaptations to the living environment to maximize safety and functionality for clients.
• Engage in continuing education and professional development to stay at the forefront of advancements in physical therapy practice.

Patient Population / Client Focus

In this role, you will primarily work with a diverse patient population suffering from various orthopedic conditions, including post-operative recovery, joint replacements, sports injuries, and chronic pain disorders. Your expertise will guide clients through the journey of rehabilitation, helping them regain strength, mobility, and confidence in their physical abilities.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A minimum of one year of experience in a physical therapy role, preferably within an acute care or home care setting.
• A current and valid physical therapy license in the state of New Jersey.
• Current CPR certification.
• A valid driver's license and reliable transportation for home health visits, if applicable.
