Acts Intuitively Therapy is seeking a dedicated Full-Time Home Care Physical Therapist to join our dynamic team in Corona, New York. Work Environment

This position is based onsite, primarily in patients' homes, allowing for personalized, one-on-one therapy sessions. The work environment is collaborative, with a strong emphasis on teamwork among clinicians. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, assessing their physical condition and functional abilities.
• Develop and implement tailored physical therapy treatment plans based on best practices and clinical guidelines.
• Deliver a variety of therapeutic modalities and interventions to improve patient outcomes, including manual therapy, therapeutic exercises, and patient education.
• Maintain accurate and detailed documentation of patient treatment progress, ensuring compliance with regulatory standards and organizational policies.
• Enable patients to achieve their functional goals by providing education on therapeutic exercises, injury prevention, and wellness strategies.
• Facilitate excellent customer service by building rapport with patients and their families, addressing concerns, and advocating for their needs.
• Collaborate with interdisciplinary teams to enhance patient care and optimize rehabilitation programs.
• Meet established productivity targets while upholding the highest standards of quality and patient satisfaction.

Patient Population / Client Focus

This role will encompass a diverse range of patients, including those with orthopedic injuries, pediatric conditions, neurologic disorders, and sports-related challenges. As a Physical Therapist at Acts Intuitively Therapy, you will have the opportunity to approach each case holistically, tailoring your treatment strategy to meet individual patient goals and needs.

Qualifications

• Completion of a Bachelor's Degree in Physical Therapy from an APTA or CAPTE accredited program.
• Current state licensure in Physical Therapy, ensuring compliance with New York State regulations.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong verbal and written communication skills, enabling effective interaction with patients and caregivers.
• Proficient in computer systems and documentation processes, with familiarity in electronic health records (EHR).
• Ability to establish and maintain trusting relationships with patients, facilitating their engagement in the therapeutic process.
• Commitment to continuous professional growth and community involvement.

Preferred Experience

While not mandatory, experience in home health care or familiarity with community-based rehabilitation practices will be advantageous. A background in treating various patient populations, especially those requiring tailored physical therapy interventions, will also be beneficial in this role.

Compensation and Benefits

Acts Intuitively Therapy offers a competitive salary range of $100,000 to $120,000 annually, commensurate with experience and qualifications. In addition to salary, we provide a robust benefits package, including:
