Work Environment

As a Physical Therapist in our home care program, you will engage with patients in their own environments. This dynamic setting allows for hands-on, personalized care tailored to each individual's unique challenges and living conditions. Working Monday through Friday, from 9 AM to 5 PM, you will have a structured schedule that promotes work-life balance while providing consistent, high-quality care. Key Responsibilities

• Conduct comprehensive evaluations to assess patients' functional abilities, identifying mobility limitations and rehabilitation needs.
• Create customized treatment plans utilizing evidence-based practices, manual techniques, and therapeutic exercises designed to enhance patient mobility, alleviate pain, and promote recovery.
• Implement interdisciplinary collaboration by communicating effectively with team members, ensuring a holistic approach to the patient's care and rehabilitation process.
• Educate patients and their caregivers about therapeutic exercises, safe movement strategies, and injury prevention techniques.
• Maintain thorough records of patient progress and treatment interventions, adhering to all regulatory standards for documentation and privacy.

Patient Population / Client Focus

Your primary focus will be on orthopedic patients, including those rehabilitating from surgeries, managing chronic pain, or recovering from injuries. You will utilize your expertise to empower these individuals, guiding them towards improved functionality and independence within their own homes. Your role is critical in helping clients regain their strength and confidence while adapting exercises to their specific needs and environments.

Qualifications

• A Master of Physical Therapy (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited institution.
• An active New Jersey Physical Therapy License, ensuring compliance with state regulations.
• A minimum of 2 years of clinical experience, ideally in outpatient settings or specialized orthopedic environments.
• Current BLS/CPR certification to ensure a safe and responsive care environment.
• Excellent communication skills combined with the ability to work collaboratively within a multidisciplinary team.

Preferred Experience

• Experience working with home care patients, particularly in a rehabilitation capacity.

Compensation and Benefits
