Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities, including the assessment of muscle, joint, and functional capabilities in accordance with evidence-based practices.
• Develop and implement personalized treatment plans to improve patients' functional mobility, alleviate pain, and ensure maximum performance based on individual capabilities.
• Supervise and mentor Physical Therapy Assistants in delivering specific components of therapy, ensuring fidelity to the treatment plan and adjusting approaches as necessary.
• Administer manual therapeutic exercises and educate clients on self-management strategies to aid their recovery process.
• Evaluate patients' home environments and provide recommendations for necessary modifications to enhance independence and safety.
• Document patient progress comprehensively through clinical records, including evaluations, progress reports, and discharge summaries, ensuring compliance with state regulations and company policies.
• Participate in interdisciplinary team meetings to collaborate on patient care strategies and enhance overall treatment effectiveness.
• Regularly communicate with physicians and case managers regarding patient status and adjust treatment plans as necessary.
• Provide guidance to clients and families on the use and care of physical therapy devices, ensuring they understand their therapy goals and exercises.
