Work Environment\n

This role will primarily be performed remotely, allowing flexibility for our providers while being supported by an outpatient office environment. Our core operating hours are Monday to Friday, 8:00 AM to 5:00 PM, with occasional evening shifts required based on patient needs. The integrated care model fosters communication and collaboration among team members, enabling you to actively engage in interdisciplinary meetings and case discussions to ensure optimal patient outcomes.

Key Responsibilities\n

• Conduct comprehensive mental health assessments by evaluating how co-occurring medical conditions impact patient mental wellness and vice versa.

• Develop individualized treatment plans that combine therapeutic approaches tailored to address both psychological and medical needs.

• Facilitate both individual and group therapy sessions, employing evidence-based techniques, to help patients enhance their coping skills and improve mental health.

• Provide crisis intervention as necessary, offering immediate support to patients experiencing acute mental health emergencies.

• Educate patients about the interplay between their physical and mental health conditions, empowering them to take charge of their treatment journey.

• Document all patient interactions and treatment plans meticulously, adhering to ethical and legal standards of confidentiality.

• Engage in ongoing professional development by participating in team meetings and reviewing literature on integrated care practices.

Patient Population / Client Focus\n

You will work primarily with a diverse clientele facing various co-occurring mental health issues and medical conditions such as chronic pain, autoimmune disorders, and substance use disorders. Your expertise in integrated care will be vital in helping these patients navigate their challenges, enhancing their overall quality of life.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.

• A current and valid license as an LCSW, LPC, or LMFT in Texas is a must.

• Demonstrated ability to understand and address the mental health needs of individuals with co-occurring medical conditions.

Preferred Experience\n

Compensation and Benefits\n

The position offers an attractive annual salary in the range of USD 75,000 to 100,000, dependent on experience and qualifications.
