Work Environment

This position is rooted in an onsite home care setting, allowing our therapists to work directly with patients in the comfort of their own homes. Key Responsibilities

• Conduct thorough evaluations of patients by reviewing their medical history, performing specific assessments, and documenting findings effectively.
• Communicate evaluation results and treatment plans clearly to patients and their families, ensuring their involvement in setting realistic and achievable goals.
• Implement treatment protocols with precision, adapting strategies based on the patient's condition and the evolving healthcare landscape.
• Collaborate with a multidisciplinary team of healthcare professionals when necessary, recognizing and documenting any changes in patient conditions to ensure timely adjustments to care plans.
• Facilitate comprehensive discharge planning and follow-up care, maintaining accurate patient documentation that reflects the course of treatment and progress.
• Demonstrate effective time management to meet productivity expectations, attending scheduled meetings punctually and maintaining a consistent schedule.
• Contribute to departmental goals by complying with all regulatory and accreditation standards and taking part in ongoing professional development.
• Actively pursue continuing education opportunities for professional growth, serving as a mentor or clinical instructor for students when applicable.
• Enhance the patient experience by being adaptable and flexible, ensuring that access to care is consistently prioritized.
• Engage in initiatives aimed at removing barriers to patient access, using collected data to inform improvements in service delivery.

Patient Population / Client Focus

This position focuses on a diverse patient population, including individuals recovering from surgery, injury, or chronic conditions. As a Physical Therapist, you will play an essential role in helping patients regain their independence and improve their quality of life through personalized rehabilitation strategies that meet their unique needs.

Qualifications

• Bachelor's Degree in Physical Therapy is required; a Doctorate degree in Physical Therapy (DPT) is preferred.
• A valid physical therapy license in the State of Florida is mandatory.
• Candidates must hold current certifications including American Heart Association's BLS for Healthcare Providers.
• While a minimum of two years of experience is preferred, we are open to candidates who are eager to learn and grow professionally.

Preferred Experience

While this role mandates at least two years of physical therapy experience, candidates with additional specialized training or certifications, particularly in home health care, rehabilitation practices, or geriatric care, are highly encouraged to apply. Familiarity with electronic medical records (EMR) systems and previous work in multidisciplinary teams will also be advantageous.

Compensation and Benefits
