Work Environment

The position is full-time, requiring 40 hours of patient care per week, with occasional weekend work. As a Home Care Specialist, your services will be delivered onsite at patients' residences. Key Responsibilities

• Conduct thorough evaluations and assessments of patients' physical needs to devise tailored treatment plans that address their specific functional limitations and disabilities.
• Collaborate with physicians and other healthcare providers to ensure continuity of care, offering insights into patients' progress and necessary adjustments to treatment plans.
• Demonstrate exemplary interpersonal skills while interacting with patients of all ages and backgrounds, adhering to high standards of customer service.
• Document patients' evaluations, treatments, and discharge plans in compliance with state regulations and professional standards, ensuring accuracy in electronic and written communication.

Qualifications

Compensation and Benefits

The salary range for this position is competitive, offering between USD 100,000 and 120,000 annually, commensurate with experience.
