Work Environment

This position is firmly based onsite within our patients' homes, allowing for tailored therapy in their familiar surroundings. The role entails working 40 hours a week, with some variable weekends as needed.

Key Responsibilities

• Conduct thorough evaluations of patients to determine their functional limitations and devise comprehensive, individualized treatment plans.
• Deliver high-quality physical therapy interventions aimed at maximizing patient outcomes and enhancing their quality of life.
• Collaborate with healthcare providers and other professionals involved in a patient's care to ensure continuity and comprehensiveness in treatment.
• Document all assessments, treatments, and progress, adhering to professional standards and regulatory requirements.
• Engage in ongoing collaboration with interdisciplinary teams, including physicians and support staff, to achieve optimal patient care.
• Participate in community outreach programs to promote the benefits of physical therapy and increase community awareness.
• Attend staff meetings and engage in continuous professional development through in-services, workshops, and additional education.

Patient Population / Client Focus

You will work with a diverse range of patients, including those with orthopedic conditions, post-surgical rehabilitation needs, and chronic pain issues. Our clients vary in age and background, necessitating a culturally competent approach that respects individual needs and preferences. The aim is always to facilitate their recovery through personalized care plans that consider both physical and emotional aspects of rehabilitation.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure in Physical Therapy is required.
• Exceptional communication skills, both verbal and written, are essential for effective patient interaction and collaboration with the healthcare team.
• Proficiency in word processing and electronic medical record systems is necessary for documentation and treatment planning.

Preferred Experience
