This position is based in a home care setting, allowing you to deliver therapy right in the comfort of your patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients in their home environments, assessing their mobility, strength, and functionality to develop tailored treatment plans.
• Implement evidence-based therapeutic interventions using techniques such as manual therapy, therapeutic exercises, and modalities to alleviate pain and enhance mobility.
• Continuously track and document patient outcomes, adjusting treatment plans as necessary based on their progress and needs.
• Educate patients, caregivers, and families on treatment protocols, strengthening their understanding and involvement in the rehabilitation process.
• Recommend and fit assistive devices to aid in patient mobility and self-care, ensuring safe and effective use.
• Stay current with best practices in physical therapy by attending workshops, participating in professional networks, and reviewing relevant literature.

Patient Population / Client Focus

You will be working with a diverse patient population that includes individuals recovering from surgeries, maintaining chronic conditions, and more. By providing care in a home setting, you can closely observe and adapt to each patient's unique environment and lifestyle, leading to more personalized and effective interventions.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid state license in Physical Therapy.
• Strong communication skills—both written and verbal—with the ability to constructively engage with patients, families, and interdisciplinary teams.
• Proficiency in basic computer skills, including documentation software.
• Flexibility to adapt to changing patient needs and work environments.

Preferred Experience
