Orthopedic Outpatient Physical Therapist

Position Overview

Swift Therapy Center is actively seeking a dedicated and experienced Orthopedic Physical Therapist to join our team in Punta Gorda, Florida. This full-time, onsite position plays a crucial role in enhancing the quality of life for our patients through comprehensive and tailored orthopedic rehabilitation services. Work Environment

This position requires working directly with patients in their homes as part of our home care facility. Our professional team engages in a collaborative and supportive work culture where open communication and shared learning are promoted. The typical working hours are Monday through Friday from 8:00 am to 5:00 pm, allowing for a balanced work-life dynamic while ensuring that patients receive consistent and high-quality care.

Key Responsibilities

• Conduct thorough evaluations of patients with functional limitations and orthopedic conditions to assess mobility, strength, and overall physical functioning.
• Develop and implement tailored treatment plans based on initial evaluations, ongoing assessments, and patient progress to ensure effective rehabilitation.
• Document patient care activities accurately and timely, including initial evaluations, treatment plans, daily progress notes, and discharge summaries, in compliance with healthcare regulations.
• Utilize evidence-based practices and advanced therapeutic techniques to maximize patient outcomes while adapting treatment protocols as necessary based on their evolving needs.
• Collaborate with other healthcare professionals, including physicians and occupational therapists, to coordinate comprehensive care and modify treatment plans as required.
• Engage in discharge planning and follow-up care, ensuring patients are equipped with the necessary tools and knowledge to continue their recovery outside of therapy sessions.
• Participate in team meetings and professional development sessions, contributing to the organization's goals and initiatives, such as the Access NRH programs aimed at increasing access to care.

Patient Population / Client Focus

The primary focus of this position is working with orthopedic patients who have sustained injuries or surgeries affecting their mobility and quality of life. This may include individuals recovering from joint replacements, sports injuries, or chronic pain conditions. The therapist will ensure each patient receives personalized attention and care tailored to their unique needs, aspirations, and recovery goals.

Qualifications

• An accredited Bachelor's Degree in Physical Therapy is required; a Doctorate of Physical Therapy (DPT) is preferred.
• Active and valid physical therapy license in the state of Florida.
• Minimum of two years of outpatient orthopedic experience, demonstrating expertise in patient evaluation and treatment.
• Current CPR certification.

Preferred Experience

• Experience with a variety of orthopedic conditions and familiarity with the latest therapeutic modalities.
• Knowledge of state and federal regulations regarding physical therapy practice to ensure compliance.
• Experience working in a home care setting is a plus.

Compensation and Benefits
