Work Environment\n

This position allows for a flexible work setting, primarily remote, enabling you to effectively balance personal commitments while fulfilling the responsibilities of a dedicated social worker. You will collaborate with a dynamic team of healthcare professionals, including nurses and caregivers, fostering a multidisciplinary approach to holistic care. The work hours generally span Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends required as client needs arise.

Key Responsibilities\n

• Conduct thorough assessments of seniors’ psychosocial needs, integrating emotional, mental, and social factors while keeping patient well-being at the forefront.

• Develop personalized care plans in collaboration with clients and their families, ensuring service delivery aligns with their specific goals and preferences.

• Provide counseling and emotional support, guiding seniors and their families through various life transitions and challenges, including chronic illness management, loss, or the complexities of aging.

• Advocate for clients’ rights and navigate community resources, ensuring access to essential services such as financial assistance, healthcare, and social activities.

• Regularly monitor client well-being through home visits to evaluate living conditions and intervene with timely assessments of any concerns or safety issues.

• Facilitate family meetings to support communication and address conflicts, enhancing the collaborative nature of care for the senior client.

• Educate seniors and their families concerning available community resources, including support groups, transportation services, and wellness programs.

• Maintain thorough documentation of client assessments, care plans, and progress notes in compliance with regulatory and ethical standards.

Compensation and Benefits\n

The compensation for this position ranges from $60,000 to $100,000 per year, depending on level of experience and qualifications.
