Key Responsibilities\n

• Conduct comprehensive initial assessments of patient mental health needs to identify individual challenges and formulate tailored treatment plans.

• Deliver evidence-based therapeutic interventions to patients via video therapy sessions, focusing on their unique situations and emotional needs.

• Collaborate with healthcare team members, including nurses and social workers, to integrate mental health considerations into the broader care plan for each patient.

• Provide ongoing support and adjustments to treatment plans based on patient progress and feedback.
\n
• Document all patient interactions and treatment progress in our electronic health record system in compliance with HIPAA regulations.

Patient Population / Client Focus\n

This role primarily involves working with adults and their families facing end-of-life issues, including managing feelings of grief, loss, anxiety, and depression. You will play a vital role in facilitating discussions around trauma, providing support for caregivers, and improving overall emotional wellness during a critical phase of life.

Qualifications\n

• A Master's degree in Counseling, Marriage/Family Therapy, or Social Work, with relevant licensure as a Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW).

• A minimum of 2 years of clinical experience in mental health counseling, with a strong foundation in treatment planning.

• Proven ability to connect with patients through virtual platforms and effectively communicate ideas and solutions.

Compensation and Benefits\n
