Key Responsibilities

• Conduct in-depth physical evaluations to assess residents' functional capabilities and devise appropriate therapy programs tailored to individual strengths and needs.
• Design and execute personalized treatment plans that address residents' mobility, strength, endurance, and functional independence, with methods including therapeutic exercises, manual therapy, and modalities as indicated.
• Track and document patient progress meticulously, making necessary adjustments to treatment plans to optimize outcomes.
• Work collaboratively with an interdisciplinary team, communicating openly about patient status and care plans while integrating feedback into therapeutic interventions.
• Deliver education to residents and their families on effective home exercise programs and lifestyle modifications to maintain improvements made during therapy.
• Ensure compliance with all professional standards and state and federal regulations governing physical therapy practice.

Patient Population / Client Focus

Our clientele primarily consists of residents within a skilled nursing facility setting, many of whom are elderly and may present with a range of conditions including post-operative recovery, chronic illness management, and mobility impairments. Our goal is to craft rehabilitation programs that address not only their physical needs but also their psychological well-being, fostering a supportive recovery journey.

Qualifications

• Active Physical Therapist licensure in the state of New York.
• Bachelor's Degree from an accredited physical therapy program; a Master's or Doctorate degree in Physical Therapy is preferred.
• Demonstrated proficiency in both oral and written communication skills.

Preferred Experience

Compensation and Benefits
