Key Responsibilities\n

• Engage directly with clients in both community settings and via telehealth to assess, diagnose, and provide treatment for individuals experiencing major mental health disorders and emotional crises.

• Develop and implement integrated care plans that ensure clients have access to comprehensive and coordinated services, facilitating effective communication among clients, their families, and interdisciplinary teams.

• Conduct thorough assessments and screenings, using evidence-based practices to inform treatment decisions and develop tailored interventions that focus on client empowerment and choice.

• Provide timely crisis intervention services, managing urgent situations with sensitivity and expertise while keeping the client’s best interests at the forefront.

• Maintain detailed records, documenting client progress and treatment outcomes in compliance with state regulations and organizational policies.

• Coordinate and participate in team meetings and case consultations to ensure a unified approach to client care and to foster an environment of learning and support.

• Participate in quality improvement initiatives and contribute to the development of agency protocols to enhance service delivery.

Patient Population / Client Focus\n

You will work predominantly with vulnerable populations in hospice care settings, supporting those grappling with severe emotional disorders and mental health issues. The patients you serve may also be dealing with terminal illnesses, representing a critical intersection of mental health support and palliative care. As such, compassion and cultural sensitivity are vital in building trusting relationships and delivering effective treatment.

Qualifications\n

• A Master’s Degree in Social Work (MSW) from an accredited institution.

• A current and valid License as a Clinical Social Worker (LCSW) in the state of Texas.

Preferred Experience\n

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 70,000 to 85,000 annually, commensurate with experience.
