Key Responsibilities\n

• Develop, implement, and monitor comprehensive treatment plans that address the unique needs of each client, utilizing evidence-based practices.

• Provide individual and group therapy sessions to assist clients in coping with their mental health challenges associated with terminal diagnoses.

• Conduct therapeutic assessments and re-assessments to ensure treatment plans remain effective and relevant.

• Deliver crisis intervention services, demonstrating a calm and skilled approach to assist clients in immediate distress.

• Build and nurture relationships with families to facilitate effective communication and support networks.

• Maintain meticulous documentation of all services provided, as required by regulatory standards and for the continuity of care.

• Engage in continuous professional development through participation in clinical meetings, in-service trainings, and quality assurance activities.

• Provide expert consultation to other healthcare professionals, ensuring a collaborative approach to client care.

Patient Population / Client Focus\n

This role is primarily focused on supporting patients in hospice care, along with their families who are navigating the complexities of end-of-life issues. Qualifications\n

• Master’s degree in Social Work (MSW) from an accredited institution.

• Active and valid Licensed Clinical Social Worker (LCSW) license in Texas.

Compensation and Benefits\n
