Work Environment

This position is fully onsite, emphasizing home care delivery. As a Physical Therapist, you will work primarily in patients' homes in the Tampa area, providing a personalized approach to rehabilitation in familiar settings. Key Responsibilities

• Develop and oversee personalized physical therapy treatment plans aimed at preventing disability and relieving pain, all while aiming to enhance muscle function and overall performance.
• Conduct comprehensive evaluations of each patient's functional abilities through various diagnostic assessments and tailor rehabilitation strategies accordingly.
• Lead and mentor Physical Therapy Assistants, ensuring the appropriate application of treatment protocols and adherence to established care plans.
• Assess the home environment for therapeutic potential, making modifications or recommendations to optimize client independence.
• Administer a variety of therapeutic interventions, including manual therapy, exercise instruction, and the application of physical agents.
• Create and maintain accurate clinical records including initial evaluations, care plans, progress notes, and discharge summaries, ensuring compliance with company policy and regulatory standards.
• Engage as an active member of the interdisciplinary team, participating in case conferences to collaboratively develop and adjust care strategies.
• Provide education and support to patients and their families regarding their therapy programs, devices, and exercises to foster engagement and compliance.
• Ensure timely submission of documentation related to orders and clinical services, adhering to branch policies.
• Perform periodic on-site assessments to revise care plans and ensure the effective use of community resources.

Patient Population / Client Focus

You will primarily work with a diverse range of patients including those recovering from surgery, managing chronic conditions, or seeking rehabilitation for musculoskeletal injuries. Qualifications

• A Doctorate degree in Physical Therapy (DPT) and a valid PT license in Florida.
• Minimum of two years of professional experience in physical therapy, preferably in a home care or acute care setting.
• Current CPR certification and valid driver's license with automobile insurance.
• Strong commitment to clinical excellence and patient advocacy, demonstrating a passion for improving patient outcomes.
• Excellent interpersonal skills and the ability to communicate effectively with patients, families, and colleagues.

Preferred Experience

Compensation and Benefits
