Position Overview

Fairview Rehabilitation Center is seeking a dedicated and compassionate Orthopedic Physical Therapist to join our team in Tampa, Florida. Work Environment

This role is mainly onsite but in a home care setting, allowing you to provide personalized therapy in the comfort of patients' homes. Our collaborative approach encourages interaction with a multidisciplinary team of healthcare professionals which creates a comprehensive care experience for our patients. We promote a healthy work-life balance with a Monday to Friday schedule, encouraging punctuality and effective time management.

Key Responsibilities

• Conduct comprehensive evaluations that include reviewing patient history, performing specific assessments, and determining recommendations. Document results meticulously.
• Communicate evaluation findings and develop individualized treatment plans in collaboration with patients and their families, ensuring their involvement in goal setting.
• Implement evidence-based treatment protocols tailored to patient needs, adjusting approaches based on ongoing assessment and best practices.
• Collaborate effectively with other healthcare providers, recognizing when adjustments to the care plan are necessary and ensuring timely consultations as needed.
• Manage discharge planning and follow-up care, ensuring continuity of care through accurate documentation including initial evaluations, daily notes, progress summaries, and discharge evaluations.
• Engage in departmental goal achievement and compliance with accreditation standards, actively participating in marketing initiatives that promote our services.
• Initiate and pursue continuing education and serve as a clinical mentor for aspiring physical therapy students.
• Contribute to enhancing patient access to care and improving customer experiences by being flexible and responsive to patient needs.

Patient Population / Client Focus

You will primarily work with orthopedic patients of varying ages who are recovering from surgery or managing chronic conditions. Understanding the nuances of each patient's situation allows for tailored care that engages and supports them throughout their rehabilitation journey.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Doctorate in Physical Therapy is preferred.
• State licensure as required for practice in Florida.
• Current American Heart Association BLS for Healthcare Providers CPR certification is essential.

Preferred Experience

While prior work experience is not mandated, demonstrated clinical knowledge and a proactive approach to patient care will be beneficial. Experience in home care settings is advantageous.
