• Conduct comprehensive assessments of seniors' psychosocial needs, factoring in emotional, mental, and social dimensions to establish a thorough understanding of each client's circumstances.

• Collaborate closely with clients and their families to develop personalized care plans that are aligned with their individual goals, preferences, and cultural considerations.

• Provide empathetic counseling and emotional support to clients, facilitating discussions around life transitions, grief, and other challenges associated with aging.

• Advocate for clients, ensuring they are aware of and can access vital community resources, support services, and health care options available to them.

• Coordinate with an interdisciplinary team to enhance care delivery, which may involve working with healthcare providers to integrate social work interventions with medical treatment plans.

• Conduct regular check-ins and virtual visits to monitor client progress, assess living conditions, and promptly identify any concerns affecting their safety and well-being.

• Facilitate communication among family members to address conflicts and ensure everyone involved is aligned with the care goals for the senior client.

• Educate clients and their families about available resources, support groups, and programs that can enhance their social engagement and individual well-being.

• Maintain meticulous documentation of assessments, care plans, session notes, and client interactions while ensuring compliance with ethical standards and confidentiality.

The primary patient population consists of seniors, many of whom are navigating complex health challenges, transitions, and emotional adjustments associated with aging. You will work closely with older adults who may face issues such as chronic illness management, caregiver stress, isolation, and mental health concerns, including depression and anxiety. • Master's degree in Social Work from an accredited institution.

• A current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

The salary range for this position is $60,000 to $100,000 per year, commensurate with experience.
