Work Environment

This position is primarily onsite within the comfort of our patients' homes, delivering care through our Home Care facility model. Our team operates from Monday to Friday, promoting work-life balance while ensuring our patients receive timely and effective rehabilitation services. Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing medical histories and performing targeted assessments to develop tailored treatment plans.
• Clearly communicate findings and treatment goals to patients and their families, ensuring they are engaged in the decision-making process regarding their care.
• Implement evidence-based treatment protocols suited to each patient's unique presentation and needs, adapting approaches based on progress and feedback.
• Consult with other healthcare professionals, when necessary, to adjust treatment plans based on evolving patient conditions.
• Maintain accurate and timely documentation of patient interactions, evaluations, and treatment outcomes, adhering to regulatory and accreditation standards.
• Participate in discharge planning and follow-up care to ensure continuity of treatment and support for patients.
• Engage in hospital and community outreach initiatives to promote our services and foster greater accessibility for potential patients.
• Seek out professional development opportunities and participate actively in continued education to enhance clinical skills and knowledge.
• Assume a mentorship role for students or new staff, fostering a supportive learning environment.

Patient Population / Client Focus

At Stay Graceful PT Clinic, our primary focus is on orthopedic rehabilitation, working with patients who may have experienced sports injuries, post-operative recovery, or chronic musculoskeletal conditions. You will actively contribute to the recovery of diverse patient demographics, tailoring approaches to their individual needs and preferences while ensuring a supportive and understanding therapeutic environment.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• A Doctorate in Physical Therapy is preferred.
• A valid state license in Physical Therapy, as required by Florida law.
• Current American Heart Association BLS for Healthcare Providers CPR certification is mandatory.

Preferred Experience

Although prior work experience is not required for this position, candidates with a background in orthopedic physical therapy or home healthcare settings will be strongly considered. We value a proactive approach to learning, where the drive to seek knowledge and enhance patient care is essential.

Compensation and Benefits
