Position Overview

Key Responsibilities

• Conduct thorough evaluations of patients, including reviewing medical histories, performing necessary assessments, and documenting findings and treatment recommendations.
• Explain assessment results and collaborate with patients and their families to set personalized treatment goals.
• Implement tailored treatment protocols based on individual evaluations, adapting strategies as necessary to improve patient outcomes.
• Consult with interdisciplinary teams to address complex patient needs, identify changes in conditions, and adjust care plans accordingly.
• Ensure comprehensive discharge planning by maintaining clear documentation of patient care activities, including evaluations, treatment plans, and progress summaries.
• Demonstrate effective time management by adhering to scheduling, maintaining productivity standards, and timely participation in meetings.
• Contribute to the development of department objectives while ensuring compliance with relevant regulations and accreditation standards.
• Engage in continuous professional development through continuing education courses and act as a clinical instructor for students.
• Promote excellent customer service by being adaptable and responsive to the needs of patients and their families.
• Actively participate in initiatives aimed at improving patient access to care, regularly engaging with CSI measurement data to inform practice improvements.

Patient Population / Client Focus

Our patient population consists of individuals across various demographics, from post-operative rehabilitation cases to chronic pain management. Qualifications

• Doctorate in Physical Therapy (DPT) with a valid state licensure to practice in New Jersey.
• Current CPR certification recognized by the American Heart Association.
• Strong communication skills and ability to work collaboratively within a team structure.
