Key Responsibilities\n

• Conduct psychosocial assessments for patients and their families during initial visits, annual evaluations, and as clinically indicated. Use established social work theories and techniques to identify barriers to care and develop appropriate intervention strategies.

• Collaborate with other healthcare professionals to create comprehensive psychosocial treatment plans, coordinating referrals to various specialty and social services, including entitlement programs to ensure holistic patient care.

• Support patients and their families in coping with environmental and psychological challenges related to illness, helping them navigate emotional distress while providing crisis intervention as needed.
\n
• Independently manage a caseload of patients with varying levels of clinical acuity, ensuring timely and responsive care that meets individual needs.
\n
• Engage in proactive case finding to identify patients with psychosocial needs, actively participating in team meetings to share insights and strategies for comprehensive care.
\n
• Offer psychoeducation to clients and families about illness, treatment options, and the adaptation process, facilitating smooth transitions from youth to adult mental health services.
\n
• Maintain accurate and thorough electronic medical records, documenting progress notes and treatment plans in accordance with department policies and health system protocols.

• Provide individual and group clinical supervision to MSWs and student interns, contributing to their professional development and ensuring high standards of care are upheld.

Qualifications\n

• Valid licensure as a LCSW, LMFT, or LPCC is mandatory.
