Key Responsibilities\n

The Licensed Mental Health Therapist will undertake a range of critical responsibilities to ensure the best outcomes for our patients, including but not limited to:

• Providing psychological support to patients and their families coping with mental health challenges, chronic illnesses, and caregiving difficulties.

• Maintaining up-to-date knowledge of local and national health care provider networks to assist in smooth transitions of care.

• Collaborating with interdisciplinary teams, including primary care providers and community resources, to create comprehensive care plans tailored to individual patient needs.

• Advocating for patients' rights, providing education to ensure informed decisions about treatment options, and facilitating access to protective services when necessary.

• Monitoring patient progress using electronic medical records (EMR) and timely updating of treatment plans based on client feedback and clinical observations.

• Participating in care conferences and transition planning discussions to ensure optimal support for patients moving between different stages of care.

• Utilizing technology to identify trends and prevent readmissions, ensuring vulnerable patients receive the appropriate level of support.

• Addressing the social and financial needs of patients and their families by connecting them to relevant resources.

• Documenting all clinical interactions and maintaining confidentiality per healthcare regulations.

• Undertaking additional tasks as assigned to support organizational goals and enhance service delivery.

Patient Population / Client Focus\n

Qualifications\n

To be considered for this position, candidates must possess the following qualifications:

• A Master’s or Doctorate degree in Clinical Psychology, Counseling, Clinical Social Work, or a relevant field from an accredited program.

• An active and independent license to practice in Oregon as an LCSW, LMFT, or LPC.

Compensation and Benefits\n
