Your primary responsibilities will include:

• Conducting thorough patient assessments, including both diagnostic evaluations and functional assessments, to create tailored treatment plans.

• Implementing targeted interventions and counseling sessions focused on therapy and skill-building to foster client growth and development.

• Consulting with primary care providers to determine appropriate referrals for specialized services when needed, facilitating a seamless care pathway.

• Monitoring patient progress through follow-up evaluations and adjusting treatment plans as necessary to meet their evolving needs.

• Developing relapse prevention plans and assisting clients in establishing coping mechanisms that promote stability and health.

• Identifying at-risk individuals and creating intervention strategies to impede deterioration of their mental health.

• Documenting client progress and assessments meticulously in treatment charts to ensure accurate records for all care provided.

• Maintaining open lines of communication with primary care providers, contributing to collaborative treatment planning.

• Facilitating behavioral change by helping clients better manage comorbid physical health issues through education and support.

• Providing self-management resources and materials that empower clients to take an active role in their recovery.

• Offering expert consultation to clinic management and collaborating with team members on behavioral health services.

Qualifications\n

The ideal candidate will possess:
