Work Environment

This position is fully onsite, allowing Physical Therapists to conduct patient evaluations and treatment interventions within the comfort and familiarity of each patient's home. The work hours are Monday to Friday from 2 PM to 8 PM, providing a balanced schedule that promotes work-life harmony while ensuring our patients receive the care they need during the afternoon and evening hours. This unique work environment fosters strong relationships with patients and their families, enhancing the overall effectiveness of therapeutic interventions.

Key Responsibilities

• Manage comprehensive care for an assigned caseload of patients, working collaboratively with physicians and the healthcare team to develop and implement individualized treatment plans tailored to each patient's needs.

• Conduct thorough assessments of patients' physical conditions to establish measurable goals and objectives, continuously evaluating progress and adjusting care plans as necessary.

• Deliver hands-on therapeutic interventions, including exercise therapy, manual therapy, and the use of modalities, to facilitate recovery and promote functional independence.

• Educate patients and their families on therapeutic exercises, equipment usage, and self-management strategies to maintain and enhance progress beyond therapy sessions.

• Organize and prioritize the caseload effectively, ensuring appropriate staffing and coordination of patient care assignments within the home care team.

• Maintain accurate and timely documentation of patient evaluations, progress notes, and outcomes to support ongoing treatment and compliance with regulatory requirements.

Patient Population / Client Focus

Your role as a Physical Therapist at Western Physical Therapy will involve working with a diverse patient population, including adults recovering from surgeries, individuals with chronic musculoskeletal conditions, and children requiring rehabilitative support. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program is required.

• A valid Physical Therapy license to practice in New York State.

• Certification in Basic Cardiac Life Support (BCLS) must be obtained within the first 30 days of employment.

• Strong interpersonal communication skills, demonstrating compassion and empathy when interacting with patients and their families.

Preferred Experience

• Previous experience in a home healthcare setting is advantageous but not required, as new graduates are encouraged to apply.

Compensation and Benefits

Western Physical Therapy offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
