Key Responsibilities\n

As a Licensed Clinical Social Worker, your responsibilities will include but are not limited to:

• Providing vital clinical behavioral health services that align with the health center’s mission, aiming for improved patient health outcomes and satisfaction.

• Assisting mental health consumers in navigating both physical and behavioral healthcare systems to achieve comprehensive integrated healthcare.
\n
• Educating patients and their families about available resources and services to foster participation in disease prevention, wellness programs, and the promotion of healthy lifestyles.
\n
• Participating in multidisciplinary treatment team meetings to discuss patient cases, contribute to care planning, and enhance collaborative treatment approaches.
\n
• Ensuring timely and clear reporting of crises and unusual incidents to the Medical Director and immediate notification of psychiatric and medical emergencies to the Chief Medical Officer.
\n
• Documenting clinical interactions meticulously in accordance with established protocols and maintaining up-to-date records as required.
\n
• Engaging in our Quality Improvement program through feedback, participation in evaluations, and attending training sessions.
\n
• Adhering strictly to all organizational policies and guidelines while working as part of an integrated healthcare team, leveraging the provided Primary Care Behavioral Health model.

• Managing evidence-based screening tools to assess patients' healthcare needs and reporting findings effectively.
\n
• Working collaboratively with patients experiencing Opioid Use Disorder as part of our Center of Excellence Team, offering tailored interventions and support.

Patient Population / Client Focus\n

Your expertise will be pivotal in supporting a diverse patient population, including individuals facing mental health challenges and those with complex medical conditions. By providing integrated behavioral health services, you will directly influence the quality of care delivered to our patients, ultimately aiming for improved outcomes across the board.

Qualifications\n

To be considered, candidates must meet the following qualifications:

• A Master’s degree in Social Work is required, along with a valid Licensed Clinical Social Worker (LCSW) credential.

• A minimum of 2-3 years of experience in clinical social work, demonstrating a proven ability to apply therapeutic techniques in a healthcare setting.

• The ability to collaborate effectively with a range of healthcare professionals, social service providers, and patients is essential.

• This role may require travel to various CHDC sites for meetings, workshops, and outreach initiatives, necessitating a valid driver’s license and the ability to travel as needed.

Preferred Experience\n

While the minimum experience required is 2-3 years, candidates with experience in medical settings such as primary care or specialty care environments will find their backgrounds especially relevant. Familiarity with various healthcare systems and integration processes will be highly advantageous.

Compensation and Benefits\n

This position offers an attractive salary range of USD 90,000 – 100,000 yearly, commensurate with experience and qualifications.
