Key Responsibilities\n

• Provide individualized counseling and crisis intervention, focusing on the assessment and treatment of emotional and behavioral disorders.

• Collaborate with physicians and healthcare team members to monitor patient progress, share updates, and articulate any specific needs, concerns, or observed challenges.

• Document clinical interactions accurately and timely, maintaining comprehensive clinical notes as per departmental policies and best practices.

• Engage in the development of personalized treatment plans, including discharge plans, based on thorough psychosocial assessments and approved social work methods.

• Foster and maintain relationships with community leaders and resource groups, representing Trusted Hands Health Center at meetings and committees addressing patients' social needs.

Qualifications\n

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Possession of an independent license such as Licensed Professional Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) is essential.

• A minimum of two years of professional experience working in a mental health counseling capacity is necessary for consideration.

Compensation and Benefits\n
