Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the strengths, needs, and challenges faced by clients and their families.

• Develop, implement, and monitor individualized treatment plans that address the emotional and psychological aspects of patients facing terminal illness.

• Facilitate individual, family, and group therapy sessions through telehealth platforms, providing therapeutic interventions aimed at improving emotional well-being.

• Serve as a liaison with community agencies to connect patients and families with vital resources such as financial support, healthcare services, and social services.

• Proactively address crisis situations, utilizing intervention strategies for issues such as mental health emergencies or domestic problems.

• Maintain accurate and timely documentation of client interactions, treatment plans, and outcomes in compliance with regulatory standards.

• Provide ongoing monitoring and follow-up with clients to ensure progress and make adjustments to care plans as necessary.

Qualifications\n

• Master’s Degree in Social Work (MSW) from an accredited program.
