Work Environment

This onsite role involves providing care in diverse home environments, allowing for a dynamic and flexible work setting. Key Responsibilities

• Evaluate and assess patient conditions to develop personalized treatment plans aimed at promoting recovery and enhancing mobility.
• Utilize various therapeutic techniques, including therapeutic exercises, manual therapy, and specialized equipment to alleviate pain and improve function.
• Monitor and document patient progress, making adjustments to treatment plans as required to ensure optimal outcomes.
• Collaborate with patients, caregivers, and families to educate them on self-care techniques and the appropriate use of assistive devices.
• Engage in continuous professional development to maintain expertise in current treatment methodologies and best practices in physical therapy.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will work directly with a diverse patient population that includes children and adults with varying needs, ranging from post-operative rehabilitation to chronic illness management. Your role is pivotal in supporting patients of all ages as they strive to regain their independence and improve their quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid state license to practice physical therapy.

• Excellent adaptability and flexibility to respond to the varied situations encountered in home care.

Preferred Experience
