As a member of our community health clinic, you will work within a remote model, allowing for flexibility while maintaining a collaborative culture. You will be equipped with a range of telehealth tools to effectively manage client interactions, assessments, and treatment planning.

Key Responsibilities\n

• Perform comprehensive psychosocial evaluations and assessments for assigned patients, utilizing strong interviewing skills and thorough report-writing capabilities.

• Develop individualized treatment plans based on thorough diagnostic evaluations and patient feedback to tailor therapeutic approaches effectively.

• Conduct various modalities of therapy including individual, family, and group sessions, ensuring adherence to state regulations and ethical standards.
\n
• Maintain accurate and detailed progress notes in alignment with program procedures to document patient developments and inform treatment adjustments.
\n
• Implement efficient discharge planning, collaborating closely with patients, families, nursing staff, and other stakeholders to ensure a smooth transition post-treatment.

• Facilitate group therapy sessions, employing group dynamics principles to engage participants and promote a supportive environment conducive to healing.

• Utilize knowledge of community resources to assist with the referral process, ensuring patients receive continued care and support beyond our clinic.

Patient Population / Client Focus\n

This role is focused on providing care to a diverse range of patients, including individuals experiencing anxiety, depression, PTSD, and other mental health disorders. You will work collaboratively with patients from various backgrounds, tailoring your approach to meet their unique cultural, social, and economic needs. Your contribution will directly impact their mental health journeys and overall quality of life.

Qualifications\n

• A Master’s degree from an accredited social work program recognized by the Council on Social Work Education.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) issued by the California Board of Behavioral Sciences.

Compensation and Benefits\n
