Job Description This full‑time position supports patients in Lake Forest, Gurnee and surrounding areas. The RN serves as a home health case manager, leading and collaborating to provide management and delivery of patient care plans. Responsibilities Lead and collaborate to provide management and delivery of patient care plans. Support patients in Lake Forest, Gurnee and surrounding areas. Consistently meet critical deadlines for charting and documentation. Self‑manage time and schedule, ensuring efficient coverage of patient visits. Qualifications RN license in the state where you work. Graduate of an approved school of practical nursing. Minimum one year of RN experience and clinical competence to deliver quality patient care. Current driver’s license with ability to spend approximately 20% of your day traveling to patient locations. Demonstrated experience with tablets, mobile phones and EMR software. Commitment to consistently meet critical deadlines for charting. Ability to self‑manage time and schedule. Successful completion of a drug screen prior to employment, which includes a background check for medical and recreational marijuana.
