Key Responsibilities\n

• Conduct comprehensive patient intakes focusing on diagnostic assessments and functional evaluations to guide treatment plans in collaboration with the primary care team.

• Implement brief, targeted interventions for patients experiencing mental health challenges, monitoring their progress, and adjusting treatment strategies as necessary.
\n
• Explore and address psychosocial issues that are impacting patients’ mental health through a structured approach that adheres to the guidelines set forth by our Agency and Federally Qualified Health Center (FQHC).

• Develop and implement relapse prevention plans, providing ongoing support to ensure patients maintain stable mental health.
\n
• Engage in collaborative discussions with primary care providers concerning the appropriate referral of patients to specialized mental health facilities as indicated.
\n
• Maintain precise documentation of patient progress, diagnoses, and treatment plans ensuring compliance with ethical standards and organizational policies.
\n
• Educate patients on self-management strategies, empowering them to take an active role in their treatment and recovery journey.
\n
• Assist patients in adhering to medical treatments initiated by other healthcare providers, offering guidance on managing medication side effects.
\n
• Participate in interdisciplinary team meetings, providing consultation about behavioral health needs and treatment strategies.

As an LCPC at Crystal Lake Families & Kids Services, you will serve a diverse patient population, including individuals and families facing various mental health challenges. Your expertise will be crucial in identifying at-risk individuals and developing effective intervention plans that address their unique psychosocial needs.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

• Current and unrestricted Licensed Clinical Professional Counselor (LCPC) credentials.

• Demonstrated knowledge of mental health assessment, treatment planning, and psychotherapeutic techniques.

• Proficiency in DSM-IV diagnoses and a solid understanding of developmental psychopathology.

• Strong communication and interpersonal skills to establish effective relationships with patients, families, and team members.

• Ability to maintain confidentiality and adherence to ethical conduct standards.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
