Work Environment\n

This position is remote, offering the flexibility to work from home while being a critical part of our hospice care team. You will engage with clients through virtual platforms and may occasionally assist in person, ensuring minimal disruption to their routine. Your work schedule will be Monday to Friday, from 8 am to 5 pm, allowing for a balanced work-life dynamic.

Key Responsibilities\n

• Develop and implement individualized care plans by coordinating and managing support packages tailored to clients’ unique needs.

• Conduct thorough interviews with clients and assess their mental health conditions, personal strengths, vulnerabilities, and specific challenges they face.

• Set measurable and achievable goals for clients, continuously reviewing and adjusting these goals based on their progress and outcomes.

• Provide crisis intervention services during emotional distress, ensuring timely support through effective communication and empathy.

• Maintain comprehensive and accurate case records to document clients’ progress and facilitate ongoing treatment adjustments as necessary.

• Participate in regular case conferences to collaboratively discuss client needs and share insights while providing essential testimony in court if required.

• Collaborate with interdisciplinary teams, acting as a key worker in connecting clients with appropriate community resources and services that enhance their recovery journey.

Patient Population / Client Focus\n

In this role, you will primarily focus on clients who are in hospice care, experiencing various emotional and psychological challenges related to end-of-life decisions, grief, and family dynamics. Your compassionate approach and understanding of complex issues such as neglect, child abuse, domestic violence, and substance abuse will be essential in providing clients and their families with the utmost support and guidance during vulnerable times.
